This page includes all of the forms and links needed for the 2024-25 Cosmic Creator Challenge including a link to the Student Description Google form, the Teacher Summary Google Form, the Student Registration form, and the Peer Critique Form. The pretest and posttest forms will be added as the Challenge contest begins at the start of September and their links sent as emails on the date you request (prior to and following your four-week project period).

Class Registration: Click on this link to fill out the registration form for each of your classes. This is your first step to begin the Cosmic Creator Challenge with your students.

Informed Consent Form: This form must be signed by a parent, the student, and the student’s teacher. These need to be physically printed and signed, then mailed to David V. Black at DVBlack@slco.org (my email at Clark Planetarium, which is a part of Salt Lake County).

Utah Rural Schools Association conference presentation: To kick off this year’s Challenge, I gave a presentation at the Utah Rural School’s Association conference in Richfield, Utah on July 17. It includes some descriptive graphics and basic results of last year’s pilot program. Anyone who attended that session or who would like to see that presentation, I have converted it into a PDF and it is linked here:


Advertisement for your Classroom: This advertisement can be printed out and posted in your classroom and provides the basic categories and contact information for the Challenge.


Rules for the Contest: This poster can be downloaded, printed, and displayed in your classroom to show the basic rules of the Cosmic Creator Challenge.
